EPC Medicare
You recently visited your doctor to discuss a chronic problem you have. Your doctor has informed you that you will be placed on an Enhanced Primary Care Plan (EPC) and that you are entitled to up to 5 “free” physiotherapy visits.

A Chronic Management Plan are a means for General Practitioners (GPs) to manage health care (including multidisciplinary care from a GP and Allied Health Services) to patients with chronic or terminal medical conditions (a medical condition that has been or will be present for a minimum of 6 months). This makes patients eligible for Medicare rebates for certain Allied Health Services through the GPs referral.
The Chronic Management Plan will be provided by your GP if you are deemed eligible for this service following a clinical judgement from the GP after considering a patient’s medical condition and healthcare needs. This program allows for a maximum of 5 Allied Health Service rebates per year.
